AFFIDAVIT OF SUPPORT AND GUARANTEE

KNOW ALL MEN BY THESE PRESENTS:

I, (Name), a citizen of (Country), of legal age, (Civil Status), with permanent address at (Address), having been sworn to in accordance with law, do hereby depose and say: 

1. I am sponsoring the visit of (Name), of legal age, Filipino citizen with Philippines passport bearing No. (Passport Number) herein after referred as the subject who is my (Relationship with the Sponsor);

2. I guarantee that I shall provide ALL financial support to pay for food, accommodations, and travel including airfare for return to the Philippines, medication, hospitalization, and other expenses, debts, and obligations incurred but not limited to immigration fines and penalties of the "Subject";

3. I guarantee that the "Subject" shall NOT in any way or manner whatsoever be a public burden or ward in (Country of destination) or any country en route from or to the Philippines;

4. I guarantee that the "Subject" shall NOT stay in the (Country of destination) for a period longer than the number of days allowed by (Country of destination) Immigration authorities;

5. I guarantee that the "Subject" is visiting the (Country of destination) entirely for tourist and recreation purposes and is NOT visiting the (Country of destination) for (1) employment (2) seek employment (3) to transit via the (Country of layover or connecting flight) to another country where deployment of Filipino nationals is restricted or where deployment requires clearance or endorsement of relevant government departments and agencies in the Philippines.
