4. In the SSS account dashboard, go to Services then Membership Records and click Submit Request for Member Data Change (Simple Correction) tab.
5. Select the Civil Status tab, and enter your new Middle Name and Last Name.
6. Upload the required documents:
- Birth Certificate
- Marriage Contract/Marriage Certificate
- Valid ID
7. Submit the changes to your account.
